Level 4 Limit Break Locations - FF7 Walkthrough
This guide details how to get every character's level 4 Limit Break as well as gives some information about leveling Limit Breaks in general and how they work. In order to learn a character's level 4 Limit Break you will need to learn every previous Limit Break for that character. Once you do this you will be able to use the item you get when completing the level 4 Limit Break quest on that character.
Every character has a level 4 Limit Break except Cait Sith, also both Cait Sith and Vincent only have 1 Limit Break per level instead of 2. That brings me to the next point, every character unlocks a new Limit Break level by defeating a certain amount of enemies. They have to land the killing blow to get credit towards their Limit Break unlock.
To learn the second Limit Break in every tier you'll need to use the first one a number of times, usually 6 - 8. If you want the exact numbers of how many kills it will take as well as how many times you'll have to use that Limit Break check out the FF7 Wikia for Limit Breaks. That has all the info you need.
The last thing I will leave you with are some random tips that I believe will be helpful for leveling up your Limit Breaks quickly.
Tip: Learn Enemy Skill Aqualung and find areas with large groups of enemies (like outside Wutai or Mideel) and lay waste to them all using Aqualung
Tip: Using a Tranquilizer on a party member when they don't have Sadness will give that character Fury. While under the effects of Fury Limit Level rises significantly faster
Tip: Manipulate enemies then use their abilities on your party to deal a lot of damage and raise Limit Levels. Serpents in Sunken Gelnika are good for this with their Aqualung ability as are the Parasites in Northern Crater with their Magic Breath ability.

Aeris - Great Gospel
This is the first Limit Break that becomes available to you in Final Fantasy 7 and technically the one that requires the most going out of your way to get. You can begin this quest shortly before going to Temple of the Ancients and once you finish Temple of the Ancients you will no longer be able to acquire this Limit Break.
For this quest you'll need to visit a man in a cave outside of Junon for some Mythril then bring it to the guy in a lone hut near Gold Saucer.

Yuffie - All Creation
Yuffie's Level 4 Limit Break All Creation is obtained by completing the Wutai Side Quest as well as the Wutai Pagoda Side Quest sometime during Disc 2 or Disc 3. The entire town of Wutai is optional and if you visit it during Disc 2, you won't be able to leave until you get your stolen Materia back from Yuffie.

Barret - Catastrophe
Barret's Level 4 Limit Break Catastrophe can be picked up in North Corel any time after completing the Huge Materia quest that takes place in this town. You don't need to save the town to get this Level 4 Limit Break, just complete the quest.

Tifa - Final Heaven
You can do this quest any time on Disc 2 after you get Cloud to rejoin your party. For this quest you'll need to put Tifa in your party and travel to her house in Nibelheim. Go upstairs to her piano and play the World Map's theme on it (combination below). Your reward for doing this will be her Level 4 Limit Break, Final Heaven.

Note: Both L1 and R1 work for this puzzle apparently.

Red XIII - Cosmo Memory
Red XIII's Limit Break is obtained by completing the Shinra Mansion Side Quest in Nibelheim at any point in the game after you first go to the town. This same side quest will reward you with the Odin Summon Materia as well as Vincent, one of the two unlockable characters in FF7.

Cid - Highwind

Vincent - Chaos
Vincent's Level 4 Limit Break, Chaos, is given to you by Lucrecia inside Lucrecia's Cave. This cave is located in the dead center of the western continent but can only be reached with a Green, Black or Gold Chocobo or after you obtain the submarine sometime on Disc 2.

Cloud - Omnislash
Cloud's Level 4 Limit Break Omnislash is found in the Gold Saucer Battle Arena as one of the more expensive prizes. You can first obtain this after you obtain the Tiny Bronco for double the price that the screenshot above shows. Later on in the game after you obtain the Highwind Omnislash drops in price.
