Judgment of the Living - Act 4 Story Quest
Judgment of the Living is the 4th main quest in Act 4 of Titan Quest: Immortal Throne and begins by speaking with Tiresias (pictured above) immediately after completing the previous quest, The Road to Hades. For this quest all you need to do is go to the Tower of Judgement and speak with the Judges there. Along the way in the areas we visit there will be NPCs that you can speak to which update this quest but they aren't required.
The next town we will be visiting is in the Upper City of Lost Souls, however, prior to reaching that town you will definitely want to complete The Stygian Lurker Side Quest. This gives you a permanent boost to your Strength, Intelligence and Dexterity on every difficulty that you complete it. You will find the monster you need for this quest in the Stygian Marsh.

Note: Just before reaching the Upper City of Lost Souls you will pass by a cave entrance which contains the Hades' Treasury Side Quest. Accepting this quest is not required to complete it, however. You can still find the quest mobs and collect the quest items without starting it.
In the Upper City of Lost Souls town (Plains of Judgement Portal) you'll find many Side Quests for you to complete. Most important of them all is The Dust of a Titan Side Quest since that will give you a permanent +2 Skill Points on every difficulty that it is completed. As for the An Inside Source Side Quest, you'll first need to complete An Invitation after which you can start the followup, An Inside Source.
Admetus Among the Dead is a Side Quest which will begin in the Prisoner's Camp which is part of the Upper Daemon War Camp area. You will need to speak with Admetus once here as well as along the way later when we make it to the Tower of Judgement. As for our primary story quest, there will be no updates along the way until you reach the Tower of Judgement which is still a decent walk away.

Just before the Tower of Judgement you will run through The Dread Path which contains the Tower of Judgment Portal, definitely something you do not want to miss. As far as the Tower of Judgment goes, there are two Side Quests which take place in here. Hades' Treasury as well as Admetus Among the Dead both have objectives you need to do in here. Other than that you can skip all of the mobs in here if you wish.
At the bottom of the Tower of Judgment you'll come across Cerberus ~ Warden of Hades (pictured below). This is the boss fight of the tower and can be quite difficult if you lack the appropriate Poison Resistance.
As you can tell from the screenshot above (and my previous paragraph) you'll need some good Poison Resistance for Cerberus. The only other damage that Cerberus does to you is Physical throughout the whole fight. Out of all Cerberus' attacks the two most deadly are Poison Spit and Poison Eruption. Eruption is what you see happening in the screenshot above where Poison explodes out of the cracks in the room.
Poison Spit is where Cerberus shoots a glob of spit at you that creates a little poison puddle on the ground. Even with max Poison Resistance this hurts a lot.
Poison Bite: A melee attack that deals heavy Poison damage too.
Poison Breath: Your standard breath attack, deals heavy Poison damage.
Poison Spit: Cerberus will hock a loogie at you and create a puddle of poison on the ground that you do not want to stand in. Deals heavy Poison damage to anyone who stands in it.
Poison Eruption: All of the cracks throughout Cerberus' room will erupt and deal heavy Poison damage to anyone hit by it.
With Cerberus defeated you'll be able to go into the adjacent room where you'll find the Judges of the tower. Speak with the middle judge to complete this quest and open up the portal to Elysium. Going through the portal will begin the next story quest of Act 4, The Battle for Elysium.
